A Near Miss: An unplanned event that did not result in injury, illness, or damage – but had the potential to do so. Only a fortunate break in the chain of events prevented an injury, fatality or damage; in other words, a miss that was nonetheless very near. (National Safety Council)
Employees are often trained to work “safely”, but are not trained to identify near misses or made aware of the positive impact that near miss reporting can have to ensure that employees are engaged in their operations.
The following are actual near miss examples submitted by ACD members confidentially for the purpose of providing lessons learned for an improved chemical industry.
